In-depth assessment of medication and the related mental health concern, regular visits and follow up, usually for 6 months, collaboration between pharmacist and physician/NP or other providers in circle of care as necessary, help with navigating mental health resources and services

Who Can Access

Anyone with NS health card who has one of more mental health or addiction diagnosis (doesn’t need to be an official diagnosis) AND a medication problem they would like to work on with their pharmacist
